Chance of hope collaborates with agencies in tailoring a transitional plan that would fulfill the State's requirements. By developing a personalized individual plan that would support a successful transition from foster care to independence. 

We offer services in Education, Employment, and Financial Literacy and provide Housing resources. Our program helps to create a smooth transition into independent living. 

Chance of hope provides an all-in-one service to agencies needing that extra support. We create a comprehensive plan that is tailored specifically to the youth's strengths and needs. The plan involves developing a long-term goal that meets their skill level and abilities which are steps necessary to access a quality of life.
